
Job Summary:
The MMI Care Coordination Social Worker has the responsibilities of a clinical service provider who provides direct services in the Care Coordination Program. The position entails working within an interdisciplinary team that may include social workers, behavioral health providers, rehabilitation providers, interpreters, medical providers, and parent resource coordinators to carry out effective care coordination activities across the MMI clinical service delivery departments and programs. The position provides direct and consultative services to patients and caregivers of individuals receiving clinical services at MMI.

Required Qualifications:
- Bachelor's degree in Social Work
- Certified or certification eligible as a Social Worker in the State of Nebraska within 90 days of hire date
- Demonstrated organizational skills with the ability to work independently and to prioritize workload and set deadlines
- Excellent communication skills necessary (oral and written). Must be able to interface in a professional manner with individuals at all levels
- Experience working with individuals with intellectual and developmental disabilities and their families and knowledge of the patient medical home model
- Ability to handle confidential matters discreetly
- Knowledge of electronic health record documentation
- Implementation of best practices related to the delivery of care coordination and case management
Preferred Qualifications:
- 1 year working with patients with IDD and/or medical social work experience
- Microsoft Excel,Microsoft Word,Microsoft Outlook
- Exprience with EPIC One Chart, Microsoft SharePoint and Microsoft Teams
- Familiarity with community health care workers and experience with individuals with intellectual and developmental disabilities.
